Create Safety Issues

Safety Issues are records of safety concerns on the project. The Quality and Safety module provides the ability to create, modify, and manage safety issues for the project. It involves regular reporting, identification, and tracking of non-conformance items from initial inspection of deficiency through to resolution. The creation of safety issues is performed at the project level as a project function.

Safety Issues can be created by an authorized user with the proper Access Control Level (ACL) in the Quality & Safety Module.

Prerequisites:

Create Safety Issues

  1. Ensure you are logged into a project.
  2. Navigate to the Quality & Safety module.
  3. Click on the New Menu.
  4. Select Safety Issues.
    The Safety Issue document screen is displayed.


5. Fill in necessary information throughout the sections. Fields with an asterisk * are mandatory in order to Save the document.

Summary:

Issues Details:

    • Title*: Alphanumeric Field.
    • Reference Number: Automatically populated consecutive Gate Three number.
    • Priority: Drop down menu selection.
    • Category:  Drop down menu selection can be modified in the Safety Issues Settings.
    • Type*: Drop down menu selection can be modified in the Safety Issues Settings.
      The safety issues workflow is based on type.
    • Reported by*: This contact defaults to the originator of the document, but can be changed by clicking the Select link.
    • Assignee*: Select the contact that this document will be assigned to by clicking the Select link.
    • Action Item Date*: Default date is today's date, but can be modified.
    • Due Date: Default date is today's date, but can be modified.
    • Description: Alphanumeric Field.
    • Location Detail: Alphanumeric Field.
    • Breakdown: Informational. Select a breakdown by clicking the Select Breakdown link.
    • Cost Code: Informational. Select a cost code by clicking the Select Cost Code link.
    • Associated Report: Informational. Attach a safety report document affiliated to the safety issue by clicking the Select Report link.
    • Financial Impact: All the fields in this section are purely informational and not mandatory.

6. Link project photos to the safety issues document in the Photos section by clicking the Add Photo(s) button.
7. Enter resolution details in the Resolution section.
8. Enter preventative actions in the Preventative Actions section by selecting the Yes radio button and filling in the additional sections. Click the Attach Files button to upload a file.
9. Click Submit.
The safety issue document is assigned to the selected contact and the document status updates to Under Review.












Safety Issues can also be created from within a Safety Report under the Related Safety Issues section by clicking the Create Safety Issues button.

Auto generated Safety Issues can also be created from within an online Safety Report. Based on set rules and conditions configurable in the Safety Settings that apply when information is completed on the online form.





Submit - Assigns and submits a new Safety Issue document through workflow. The status will change to Under Review.

Save as Draft - Saves the Safety Issue document as a draft.

Delete - Removes a Safety Issue document. Only applicable on draft Safety Issue documents.

Cancel - Cancels Safety Issue document and/or any modifications made.
